POSITION OVERVIEW
Residential Case Manager
Our Residential Case Manager will work out of our Albany, NY campus and will be responsible for supporting clients in the Residential System while performing duties in collaboration with a clinician.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Arrange and attend home visits
- Prepare transition plans
- Coordinate Independent Living Services
- Act as the primary contact between Schools and Residential
- Coach family visits
- Complete various assessments, including FASP and SPR
- Complete accurate case notes and timely input the notes into Connections database
- Provide transportation for clients
- Maintain contact with DSS and family
- Function as a contributing member of the interdisciplinary treatment team
- Attend dorm meetings
- Attend treatment review meetings
EDUCATION and/or EXPERIENCE
Requires a Bachelors degree or commensurate level of education and related experience.
Experience with individuals in care systems preferred.
Drivers license is required for this position.
Work requires 25 50% community travel/visits.

As a Director of CSE and School Psychology, you will direct and oversee the completion of intellectual, academic, and social emotional assessments and re-evaluations of students referred to the Committee on Special Education (CSE) at the School at Northeast and Neil Hellman School. POSITION SUMMARY
Provide daily therapeutic care and programming according to Agency and other governing bodies (LDSS, OCFS, CSE, etc.) standards and requirements for adolescent clients who are emotionally disturbed.

As a Residential Program Manager, you will manage and oversee child care and milieu activities across the residential system, promoting a therapeutic environment that supports active permanency planning and work. You will report to the Residential Executive Program Director and will work onsite.

Per Diem Clinician, Permanency Resource Center (PRC)
As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicated to individuals, families and groups in a Permanency Resource Center. This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases. Therapy is to be provided in the community or in office, and is paid for by the program, not through insurance reimbursement. Northern Rivers Family of Services seeks a leader for the Youth ACT Crisis Response program in Schenectady, NY. Seeking an experienced professional who will develop and supervise a multi-disciplinary team that provides services to youth and families with the highest level of need in the community. The over-arching goals of the program are to prevent out-of-home placement, in-patient hospitalization, and involvement with the child welfare system by expanding services to youth ages 10 to 21 with significant psychiatric needs, who are at risk of institutional level of care, and who require intensive interventions to support the child and family’s complex needs.
The Director will also have the opportunity to develop their team’s skills to maximize their impact on the individuals and communities the Youth ACT program serves while also providing clinical and administrative oversight to the program in collaboration with the NYS OMH and NRFS leadership.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Interface with county mental health leadership to coordinate admissions to Youth ACT and successful discharges to lower levels of care
